Course Review Form (Also used for new course proposals): Faculty mentors should complete this form when a high school requests to offer a CITS course not previously offered at their school, when a high school teacher alters their existing course syllabus, when the LSC course outline has changed, or when a new mentor or new teacher is assigned to a course. A complete and thorough review of the course, the syllabus, and course assignments should accompany the form. Complete the form fully, send a copy to the high school teacher, and submit to the CITS Coordinator. See page 7 of the Faculty Mentor Handbook below for more details.

Site Visit Form: Faculty mentors should complete this form for each course and each visit. Complete the form fully and submit to the CITS Coordinator. See page 7 of the Faculty Mentor Handbook below for more details.

CITS Mentor Communication Log: Use this form to track communication with your mentee(s). Submit to the CITS Coordinator at the end of each academic year.

CITS Professional Leave Request Form: Faculty mentors should complete this form prior to each visit. Complete the form fully and submit to the CITS Coordinator, who will route the form to Melissa Leno for approvals (rather than your dean).

Expense Reimbursement Form: Complete this form after travel. Submit to the CITS Coordinator with an email confirmation of your visit and a map printout.
