  • Course Review Form (Also used for new course proposals): Faculty mentors should complete this form when a high school requests to offer a CITS course not previously offered at their school, when a high school teacher alters their existing course syllabus, when the LSC course outline has changed, or when a new mentor or new teacher is assigned to a course. A complete and thorough review of the course, the syllabus, and course assignments should accompany the form. Complete the form fully, send a copy to the high school teacher, and submit to the CITS Coordinator. See page 7 of the College in the Schools Faculty Handbook for more details.
  • Site Visit Form: Faculty mentors should complete this form for each course and each visit. Complete the form fully and submit to the CITS Coordinator. See page 7 of the College in the Schools Faculty Handbook for more details.
  • CITS Mentor Communication Log: Use this form to track communication with your mentee(s). Submit to the CITS Coordinator at the end of each academic year.

Travel and Reimbursement Forms and Links

  • CITS Travel Process: See page 8 of the Faculty Mentor Handbook below for more details.
  • CITS Online Leave Form: New! Faculty mentors should submit this online form prior to each visit. Submission of this form will notify the CITS Coordinator, Director of Admissions, and your academic dean that you plan to travel on college business.
  • Expense Reimbursement Form: Complete this form after travel. Submit to the CITS Coordinator with an email confirmation of your visit and a map printout.
