Report an injury incident:
Employee or Student Employee Injury:
- You have 24 hours to submit the Employee Work-Related Injury, Illness or Incident Report and send to your supervisor.
- Supervisors must log into the Supervisor Toolkit on the portal to fill out the form.
- If you have questions, or need assistance, please contact the LSC Human Resources Office.
Student or General Public Injury:
- You have 24 hours to fill out the General Incident Report form.
- Students, check your LSC email for follow-up information. General Public will be notified by your personal information listed in the reporting form.
