Authorization for release of information (this is necessary for faculty to complete recommendations for jobs or scholarships).
Speaker Evaluation Form – use to evaluate guest speakers; may also be used to evaluate student presentations, including classroom and outreach
Group Presentation Rating Form – for outreach projects (1 per student)
Project Outcome Report – for outreach projects (1 per group)
Exam Analysis (blank form for exams, quizzes, etc).
