ADA/Disability
ADA Request Form 1. To initiate the process, employees will complete the ADA Request Form and return it to the Human Resources Office. Employees may also schedule and appointment with the ADA Coordinator before completing the form to clarify the process and ask questions. 4. The next step is to identify potential accommodations and assess the effectiveness of each. The ADA Coordinator may seek technical assistance from the State ADA Coordinator or the Director of Disability Services for the college. 1. Applicants inform the Human Resources Office of their request for an accommodation in writing or verbally. When a request is received from a job applicant, the Search Committee Chair and the ADA Coordinator from the Human Resources Office will discuss alternatives with the applicant and will make a decision regarding the request. If approved, the ADA Coordinator and Search Committee Chair will make sure the accomodation is provided. 2. If the request is approved, the supervisor will make sure that the accommodation is provided. If the request is not approved, the ADA Coordinator will inform the applicant in writing within 5 days. |
